Scheduling Appointments
To schedule an appointment, please call our office Monday-Friday between 10:00 am and 6:00 pm. Our appointment staff may ask you about the nature of your visit in order to book the most appropriate appointment for you. Another option is to click on the link to Zoc Doc to schedule an appointment at your convenience 24/7 with one of our providers.
Medical Records
The Augury for Women’s Health and Midwifery is fully compliant with the Affordable Care Act and provides patients with 24/7 online access to their own medical records. All patients have a personalized HIPAA compliant user name and password which will be initially generated by our staff upon check-in. Patients will then be able to log online, change their password, and obtain access to their health records via the EMR portal system. For hard copies of their medical records, a release of medical record information consent must be signed in person with identification produced.
Prescriptions
The Augury participates in electronic prescribing which allows us to instantly send prescriptions to the patient’s choice of pharmacy. Patients are highly encouraged to identify their pharmacy of choice to our office staff at check-in to expedite this process. Necessary pharmacy information includes the name, address, and phone number. If a patient does not have a pharmacy already chosen, we can help you with information bout the closest pharmacy to your home or work place.
Payments
Co-payments or all-cash payment can be made via cash, personal checks, or credit card (Visa, Mastercard, and American Express).
